First Visit Guide

To help make your first visit with us effective and productive, there are several items that you should bring with you. Please arrive 45 minutes before your scheduled appointment time. Allow 2-3 hours for initial visit. This helps us make sure that your first visit with us is thorough and complete.

What should I bring on my first visit?

  • Please complete the New Patient Form via the portal
  • All Spine related MRIs, CT scans, myelographies, including reports (paper reports only are not sufficient)
  • Any spinal surgery operative reports
  • Insurance cards: major health plan, supplemental insurance
  • Driver’s license or other photo identification

Patient Responsibilities

  • Provide accurate information
  • Taking a studious approach to learning about your medical condition
  • Weighing the risks and benefits of your various treatment and potential outcomes
  • Developing realistic expectations
  • Making the final decision about which treatment option is best for you
  • Supporting your health care team by faithfully following their instructions and working at recovering from your condition, including making lifestyle changes if necessary

Appointment Policy

  • Please give us 24 hours cancellation notice to avoid a charge of $40.
  • We try our best to accommodate late arrivals, however, if you arrive more than 15 minutes late to your appointment, we may have to reschedule. 
  • Should you need to cancel in less than 24 hours, we will charge a fee.
